Nhập các thông tin cần theo dõi

Trường Điểm trọng số là tổng điểm trọng số ở phần nhập chi tiết

Ở phần chi tiết:

Nhập các chỉ tiêu muốn đánh giá, điểm trọng số của từng chỉ tiêu

Đặt mức điểm đánh giá tối thiểu và tối đa
